BackgroundThe principles of informed consent, confidentiality and privacy are often neglected during patient care in developing countries. We assessed the degree to which doctors in Lahore adhere to these principles during outpatient consultations.Material & MethodThe study was conducted at medical out-patient departments (OPDs) of two tertiary care hospitals (one public and one private hospital) of Lahore, selected using multi-stage sampling. 93 patients were selected from each hospital. Doctors' adherence to the principles of informed consent, privacy and confidentiality was observed through (...) client flow analysis performed by trained personnel. Overall patient perception was also assessed regarding these practices and was compared with the assessment made by our data collectors.ResultsSome degree of informed consent was obtained from only 9.7% patients in the public hospital and 47.8% in the private hospital. 81.4% of patients in the public hospital and 88.4% in the private hospital were accorded at least some degree of privacy. Complete informational confidentiality was maintained only in 10.8% and 35.5% of cases in public & private hospitals respectively. Informed consent and confidentiality were better practiced in the private compared to the public hospital (two-sample t-test > 2, p value < 0.05). There was marked disparity between the patients' perspective of these ethical practices and the assessment of our trained data collectors.ConclusionObservance of medical ethics is inadequate in hospitals of Lahore. Doctors should be imparted formal training in medical ethics and national legislation on medical ethics is needed. Patients should be made aware of their rights to medical ethics. (shrink)

Now more than ever the role of the other has been put into question and marginalized in a redefinition of an “American national self-protective identity” in the current post election climate. In philosophical terms, an identity of a radical other- implies that any change, any difference, any impurity can be conceived as posing a threat to identity. If a specific group of people is identified as preventing the self from being what it ought to be, the other is identified as (...) a security threat. One option is to willingly conform to the assigned role as a threat. The opposite option I argue in my paper is one that can be achieved through the lens of a cosmopolitan framework of hospitality. By exploring the writings of French philosopher, feminist and psychoanalyst Luce Irigary’s cosmopolitan pedagogy of difference rooted in the concept of hospitality from the point of view of education, I hope put forth an alternative phenomenological pedagogy, one that connects issues of positionality with those of an embodied anti-racist philosophy of difference. By using case studies from my own classroom, I hope to elucidate how educators can implement an anti-racist pedagogy in the current post election climate, by creating spaces for dialogue where students are engaging in an authentic discourse on the nature of their positionality. The increasing global demand for energy necessitates devoted attention to the formulation and exploration of mechanisms of thermal heat exchangers to explore and save heat energy. Thus, innovative thermal transport fluids require to boost thermal conductivity and heat flow features to upsurge convection heat rate, and nanofluids have been effectively employed as standard heat transfer fluids. With such intention, herein, we formulated and developed the constitutive flow laws by utilizing the Rossland diffusion approximation and Stephen’s law along with the MHD (...) effect. The mathematical formulation is based on boundary layer theory pioneered by Prandtl. Governing nonlinear partial differential flow equations are changed to ODEs via the implementation of the similarity variables. A well-known computational algorithm BVPh2 has been utilized for the solution of the nonlinear system of ODEs. The consequence of innumerable physical parameters on flow field, thermal distribution, and solutal field, such as magnetic field, Lewis number, velocity parameter, Prandtl number, drag force, Nusselt number, and Sherwood number, is plotted via graphs. Finally, numerical consequences are compared with the homotopic solution as a limiting case, and an exceptional agreement is found. (shrink)

In developed economies, powerful legislative and regulatory frameworks, for people with disability over the last five decades, have provided major motivation for business compliance with disability in the workplaces. However, developing economy like India is marked by emergent disability legislation, weak institutional enforcement and an evolving disability rights movement. In the absence of strong institutional expectations, the private sector’s role in mainstreaming the disability agenda has been largely an act of voluntary participation. Drawing upon an in-depth, multilevel, cross-functional qualitative study (...) of four Indian information technology sector companies, this paper explores why these companies engage in pro-social corporate behaviour in favour of disability. The study locates itself in the context of conceptualization of PWD as employee stakeholders and the literature on strategic CSR. The findings reveal that strategic factors promote voluntary business engagement with disability at workplaces and contribute to understanding of workplace integration of minority employees. Bioethics and health researchers often turn to Islamic jurisconsults (fuqahā’) and their verdicts (fatāwā) to understand how Islam and health intersect. Yet when using fatwā to promote health behavior change, researchers have often found less than ideal results. In this article we examine several health behavior change interventions that partnered with Muslim religious leaders aiming at promoting organ donation. As these efforts have generally met with limited success, we reanalyze these efforts through the lens of the theory of planned behavior, (...) and in light of two distinct scholarly imperatives of Muslim religious leaders, the ʿilmī and the islāhī. We argue for a new approach to health behavior change interventions within the Muslim community that are grounded in theoretical frameworks from the science of behavior change, as well the religious leadership paradigms innate to the Islamic tradition. We conclude by exploring the implications of our proposed model for applied Islamic bioethics and health research. (shrink)

An examination of seventeenth- and eighteenth-century French history indicates that the relationship between the Jesuits and Quietism was shaped by politics as well as by concerns of theological orthodoxy. During the late 1690s, the Jesuits championed François Fénelon accused of Quietism at the same time as they spearheaded an attack against Quietism in Burgundy, emphasizing crimes of spiritual incest or the abuse of clerical authority. Such ambiguity indicates that the Jesuits were motivated by a desire to consolidate political power in (...) Louis XIV... trades court. However, the fusion of Quietist heresy, charges of spiritual incest, and political gamesmanship would ultimately make the Jesuits themselves vulnerable to claims of heresy and abuse when the Girard/Cadière affair became a national scandal in 1731. This essay argues that this disquiet over clerical behavior and power was articulated in a changing political culture between the late seventeenth century and the 1730s. Growing dissatisfaction against the crown established a new political consciousness, one that regarded the politics of secrecy as problematic if not outright illegitimate. The secrecy of the confession, the emphasis on interiority seemingly at the expense of morality, and the enigmatic language of mysticism, all associated with Quietism played into fears of clerical (or “Jesuit”) cabal and conspiracy. When Jesuit opponents linked the order to Quietism, they presented the Jesuits as threats to an emerging set of political values in which legitimate authority was transparent and open while illegitimate power operated in the shadows. The epistemological foundation of unity of knowledge is used to formulate a system-model of participatory socioeconomic development. The micro-properties of such a participatory development approach are deeply ethical in nature. In order to bring out the endogenous role of ethics derived from the moral law in reference to the epistemic foundation, and thereby explain their impact on the socioeconomic development experience, the methods of topological space and topological mappings are found to be appropriate for formalizing the complex nature of participatory (...) dynamics. They emerge in the midst of institutional and organic interaction, integration, and evolution. The application of topological methods to participatory socioeconomic development thus opens up a field that is rarely used by mathematical economists. Van Kley, leading specialist on religion and politics in the Old Regime and the French Revolution, these essays examine how Jansenist belief shaped enlightenment ideas, cultural identities, social relations and politics in France throughout the long eighteenth century. Van Kley's work has invited scholars to think beyond the traditional parameters of the Enlightenment and to consider how religious faith functioned in the broader context of Old Regime, Revolutionary, and post-Revolutionary France. In different ways, each (...) essay challenges the idea of an inherent opposition between faith and Enlightenment, which likewise equates modernity with secularization. The authors within this volume address two main questions. Firstly, how did religious belief continue to shape identities and experiences in the long eighteenth century? BackgroundAdverse Childhood Experiences have been associated with long-term physical and mental health conditions, toxic stress levels, developing unstable interpersonal relationships, and substance use disorders due to unresolved childhood adversities.AimsThis study assessed the perspectives of mental health providers regarding their adult patients’ coping with ACEs during COVID-19 in Houston, Texas. Specifically, we explored how individuals with ACEs are coping with the increased stresses of the pandemic, how MHPs may provide therapeutic support for individuals with ACEs during this pandemic, pandemic-related challenges of (...) accessing and utilizing mental health services for individuals with ACEs, and the awareness and treatment of ACEs among MHPs.MethodsTen in-depth semi-structured virtual interviews were conducted with licensed MHPs from November 2021 to April 2022 in Houston, Texas. Interviews were coded and analyzed for emerging themes through an inductive open coding approach to discover insights regarding coping with ACEs during COVID-19.ResultsFour key themes experienced by individuals with ACEs emerged from the MHP interviews: Maladaptive emotional dissonance and coping outlets during the pandemic, Difficulties with social connectedness and significance of social support, Heightened daily life stressors and coping with the ongoing disruption of the pandemic, and Changing interactions with the mental health system. Themes from this study highlighted that resilience, seeking treatment, and strong social support can help develop healthy coping strategies among individuals with ACEs.ConclusionThis study may help inform best clinical practices to develop interventions and policies regarding ACEs such as a resilience-promotion approach that targets all the socio-ecological levels. In addition, findings highlight the synergy of psychotherapeutic and pharmacological management via tele-health modalities, in helping individuals with ACEs continue receiving the care they deserve and need during a persistent pandemic and an uncertain future. In covering this extension of the field due recourse is made to the epistemological basis of theory construction. When so comprehended, information technology becomes a philosophical inquiry on a variety of social, scientific and technological issues. A new idea that we refer to as neuro-cybernetics is born. The term neuro-cybernetics is used to delineate the epistemological field of system and cybernetic study. The above-mentioned phenomenological (...) or the epistemic model of a cybernetic and system type is applied to flood control problem in Bangladesh. This example presents an application of the cybernetic model to a physico-human problem. This is the nature of socio-scientific system. This book investigates morality in a socio-scientific worldview, examining the epistemology of existence in conjunction with Islamic monotheistic law to generate a world-system that governs action and reaction in the context of a variety of cognitive and social environments.

In this paper, we employ what we term as ‘the ethical dimension in political market orientation ’ framework to underline how an integration of information from relevant stakeholder groups can inform the formulation of market-oriented, yet ethical policies. Against the backdrop of India’s Look East Policy, we undertake a critical analysis of historic economic data from 1980 to 2014 in the states of Arunachal Pradesh, Assam, Manipur, Meghalaya, Mizoram, Nagaland and Tripura, often termed as the Seven Sisters because of their (...) interdependence. Our analysis of the data, using time-series methods, reveals that policy changes ensuing from the economic reforms of 1990s and the initiation of the LEP in 1991 have resulted in regional income convergence in North-Eastern India. A key contribution of our work emanates from the fact that we determine two structural breaks endogenously from long time-series data, thereby identifying three distinct policy regimes, rather than specifying exogenous breaks based on historic and theoretical conjectures. Overall, our work offers both conceptual insights into processes that enable ethical and inclusive policy making, as well as empirical evidence emerging from stochastic convergence of incomes per capita for the Seven Sister states in the North-Eastern region in India, to illustrate how regional economic inequality can be reduced through targeted market-oriented policies. The solipsist, on the analogy of our dream-experience, imagines a higher mode of selfhood or spirit to whom the world is like a dream; his own self is a lower or deluded mode of this selfhood and to it the world appears as real. Thus objectivity appearing to the lower self is illusory and contingent, not ultimate. This analogical argument for a higher self, as against an alien God, has this counter-argument. In dreams I have unpleasant experiences because of certain (...) external causes, such as physiological and psychological. But what compels my higher self to have the unpleasant dreams that are my waking experiences of life? In other words, why cannot my higher self have nothing but rosy dreams when there cannot be anything else than itself to force on it bad ones? The answer to this is that in our dreams and fantasies we do not have unpleasant experiences due to external causes only, but also due to our inherent love of experience for its own sake. Our dreams are our wish-fulfilments too. We love to have all sorts of experiences, to taste all kinds and shades of emotion. Thirst for passions, curiosity after the unknown, and adventure for the strange and the new mark our empirical self. So it is not absurd to conceive the higher self fashioning its dreams, our waking experiences, as a mixture of pleasant and unpleasant elements rich in variety and complexity. And just as my empirical self is said to enjoy the experiences of my dreams, so this higher self may be said to enjoy his dreams, my waking experiences. (shrink)

The National Blood Policy in India relies heavily on voluntary blood donors, as they are usually assumed to be associated with low levels of transfusion-transmitted infections . In India, it is mandatory to test every unit of blood collected for hepatitis B, hepatitis C, HIV/AIDS, syphilis and malaria. Donors come to the blood bank with altruistic intentions. If donors test positive to any of the five infections, their blood is discarded. Although the blood policy advocates disclosure of TTI status, donors (...) are not, in practice, informed about their results. The onus is on the donor to contact the blood bank. Out of approximately 16 000 donations in the past 2 years, 438 tested positive for TTI, including 107 for HIV. Only 20% of the donors contacted the blood bank; none of them were HIV positive. Disclosure by blood banks of TTI status by telephone or mail has resulted in serious consequences for some donors. Health providers face an ethical dilemma, in the absence of proper mechanisms in place for disclosure of test results, regarding notification to donors who may test positive but remain ignorant of their TTI status. Given the high cost of neglecting to notify infected donors, the authors strongly recommend the use of rapid tests before collecting blood, instead of the current practice, which takes 3 h to obtain results, and disclosure of results directly to the donor by a counsellor, to avoid dropouts and to ensure confidentiality. Living Dangerously: Multiculturalism and the Politics of Difference, Henry Giroux critically examines the emphasis on “clarity” in educational discourse, the best known advocate for which is Michael Apple. Giroux points out that a new generation of social critics, particularly in feminist theory, literary studies, post-colonial analysis, and Afro-American cultural criticism, has broken with traditional conventions that call for writing in a clear, unambiguous discourse. In contrast to Apple’s interpretation of “clarity” in language, the present paper will emphasize Giroux’s claim that (...) educators need to center their discussion of language around a politics of difference that allows teachers and students to acquire the knowledge and skills necessary to govern and shape society rather than be relegated to society’s margins. This paper will argue for the development of methods of articulating how social locations shape various social and intellectual perspectives. Education for critical consciousness should focus on the links between the historical configuration of social forms and how these links work subjectively. (shrink)

The development of action representation during adolescence was investigated using a visually guided pointing motor task to test motor imagery. Forty adolescents and 33 adults were instructed to both execute and imagine hand movements from a starting point to a target of varying size. Reaction time was measured for both Execution and Imagery conditions. There is typically a close association between time taken to execute and image actions in adults because action execution and action simulation rely on overlapping neural circuitry. (...) Further, representations of actions are governed by the same speed-accuracy trade-off as real actions, as expressed by Fitts’ Law. In the current study, performance on the VGPT in both adolescents and adults conformed to Fitts’ Law in E and I conditions. However, the strength of association between E and I significantly increased with age, reflecting a refinement in action representation between adolescence and adulthood. (shrink)

Background With growth of genomics research in Africa, concern has arisen about comprehension and adequacy of informed consent given the highly technical terms used in this field. We therefore decided to study whether there are linguistic and cultural concepts used to communicate heritability of characters, traits and diseases in an indigenous African population. Methods We conducted Focus Group Discussions among 115 participants stratified by sex, age and socio-economic status and Key Informant Interviews among 25 stakeholders and Key Opinion Leaders among (...) Yoruba living in Ibadan, Nigeria. We used Atlas-ti v.8.3.17 software to analyze the data, using thematic approach. Results The study participants identified several linguistic and cultural concepts including words, proverbs, and aphorisms that are used to describe heritable characters, traits and diseases in their local dialect. These included words that can be appropriated to describe dominant and recessive traits, variations in penetrance and dilution of strength of heritable characteristics by time and inter-marriage. They also suggested that these traits are transmitted by “blood”, and specific partner’s blood may be stronger than the other regardless of sex. Conclusions Indigenous Yoruba populations have words and linguistic concepts that describe the heritability of characters, traits and diseases which can be appropriated to improve comprehension and adequacy of informed consent in genomics research. Our methods are openly available and can be used by genomic researchers in other African communities. (shrink)

By drawing on the works of Jean-Paul Sartre, Paulo Freire, and Merleau-Ponty, Professor Rasheed analyzes how Greene's work represents an advance in existentialist discourse via her interpretation of concepts, such as choice, freedom, and possibility within an educational setting.

Purpose: This research study aims to examine the relationship between perceived overqualification and turnover intention among employees with a focus on understanding the mediating role of employee wellbeing. Perceived overqualification, where employees feel that their qualifications exceed the requirements of their job, has been identified as a potential contributor to turnover intention. However, the role of employee wellbeing in this relationship remains relatively unexplored particularly in a context characterized by large supply of graduates outnumbering the demand for them. Design/Methodology/Approach: Through (...) a quantitative research design, data is collected from clerical employees in nationalized banks in the southernmost State of India. The proposed model was tested among these where the jobs are characterized by opulent extrinsic attributes. The data analysis has been done using factor analysis, reliability and validity analysis and analysing model fit indices through structural equation modelling. Findings: The results showed that the positive association between perceived overqualification and turnover intention is mediated by employee wellbeing. The job characteristics of the nationalized job were found to ameliorate the negative perceptions associated with perceived overqualification. Originality/Value: This study brings out the significance of employee wellbeing over perceived overqualification in affecting turnover intention. Hence it is an indicator to the employers to focus on factors that can enhance employee wellbeing while framing human resource policies and thereby reducing turnover intention among them. The findings of this research can enrich human resource management strategies and interventions aimed at improving employee retention and organizational performance. (shrink)

This essay presents some preliminary thoughts about the linkages between current human rights universalism and the practice of violence in the form of wars and interventions. I draw three parallels that may help us think about the current wars on terror and in Iraq. The first parallel concerns the progress of liberal universalist thought from the Enlightenment period in which a concern for rights coexisted with the justifications for imperialism. In the current era the succeeding line of universalist thought is (...) that of human rights which similarly coexists with the overt and tacit support for violence that deprives some humans of their lives.The second parallel concerns the use of national identity. In the imperial era, the justification for rights either given or withheld was closely linked to constructions of national identity. Similarly, today there is a resurgence of nationalist discourse in which the construction of U.S. national identity is used to justify the violence that is done against Iraqi citizens. This discourse which constructs the U.S. as ontologically civilized and the Iraqis as barbarians is used to justify the violence that is done to them.Finally, the last parallel concerns violence in general. During imperialism, the scrutiny for acts of violence was borne largely by the native. Because he was constructed as a barbarian, his violence was made far more obvious as further evidence of his lesser development. In the present circumstances, a similar scrutiny is borne by the Iraqi insurgent while the violence of the coalition forces remains veiled beneath euphemisms like collateral damage. The torture scandal at Abu-Ghraib presented an opportunity to reverse the gaze but because of its interpretation as an aberration that falls squarely outside the ``normal" and the failure to widen the debate to other violence, this opportunity was largely lost.These three parallels taken together suggest that the old liberal hegemonic order of imperialism with its conflicting narratives of rights and oppression has been carried forward and sublimated into a human rights regime. And human rights is now deployed to justify violence against ``human rights abusers." Because of this continuity, there is a need to create a new universalism born organically from the struggles of subordinated peoples that eliminates old-order imperialist justifications for the oppression of Others while claiming to support human rights. Australia and India have had few reasons in the past to develop systematic and significant levels of economic engagement. This was due to very different positions they have held in the world-system since the Second World War. De-colonization, the fall of the British Empire, the weak status of the British Commonwealth, and the realpolitik of the Cold War saw India and Australia located on different parts of the geo-political and economic world map with small demographic and cultural flows, and insignificant (...) economic trade. Both countries developed similar economic policy regimes that were essentially state-led nationalist projects of economic development with concomitant policies of import-substitution, local industry-subsidization, highly-regulated financial systems, and high tariffs. The last quarter of the 20th century saw a radical revision of both nations’ economic strategies, with Australia moving first to drop many of its trade barriers in the 1970s and ’80s. It is now one of the most open economies in the world. India’s liberalization programme commenced much later in 1991 but nonetheless has had a dramatic impact on its economic fortunes and growing status in the world economy. With these changes there are increasing opportunities for bilateral trade and a greater economic enmeshment in regional engagements and alignments in the Indian Ocean and in wider Asian fora. One significant indicator of change in growing Australia-India economic engagement is to look at Foreign Direct Investment (hereafter FDI). Currently, the movement of FDI between these two countries is still not very large but has a strong potential to grow over the short to medium term. This paper looks at the future prospect of this growth and asserts that, by engaging in areas of comparative advantage, it will benefit both national economies. Moreover, economic flows are also indicators of great social and cultural traffic. The movement of FDI between the two countries will not only encourage greater flow of peoples, especially outward migration from India to Australia, but also trigger more Australian expatriates living in India (from a very low base). This study examines the relationships between a company''s emphasis on discretionary social responsibility, environment, and firm performance. It tests the proposition that environmental munificence and dynamism moderate the relationship between discretionary social responsibility and financial performance. Social responsibility was measured with a three-item scale in a sample of 62 firms using a questionnaire. Environmental munificence and dynamism were measured using archival sources as was financial performance (return on assets and return on sales). The results of moderated regression analyses and subgroup (...) analyses found a significant moderating effect of environment on the social responsibility-firm performance relationship. Discretionary social responsibility contributes to firm performance in environments that are dynamic and munificent. (shrink)
